    to do is determine if your previous antidepressants were given adequate trials That means at least eight and preferably 12 weeks of treatment on at least a moderate dosage It is important to stick with an antidepressant treatment even if you have side effects when you start If side effects persist or interfere with your functioning you should bring them to the attention of your doctor for assessment and monitoring There are other avenues to explore as well PHYSICAL ILLNESS can make it harder to recover from depression A thorough medical evaluation might reveal an underlying medical condition that must be dealt with SUBSTANCE ABUSE is another common reason why depression does not improve with treatment The two conditions must be addressed together MAJOR LIFE STRESSORS can be a contributing factor Psychotherapy and or stronger coping skills can be extremely helpful in this case ANOTHER PSYCHIATRIC ILLNESS may be at work Sometimes people with TRD actually have an underlying bipolar disorder that is difficult to identify Bipolar disorder is generally characterized by periods of elevated mood and energy along with periods of depression However some people will view those periods of euphoric well being and increased productivity as their normal self rather than a hypomanic mood state Or someone may experience irritability instead of happy moods and this irritability can be difficult to separate from depressive symptoms Talking to supportive family and friends can help you figure out if these alternating mood patterns are recurring features of your illness Other important considerations include having a family history of bipolar disorder decreased sleep and increased activity during periods of irritability or elevated mood having a quick initial response to antidepressants that then fades and or feelings of agitation and increased irritability with antidepressant treatments If you can relate to any of

    reasons couldn t stop taking but it was also a way to escape I guess or deal with an empty feeling the times you talk about right A Oh yeah My whole life I had this empty feeling inside me I didn t feel like I was good enough I had a lot of pain yeah Q It seemed from your book that maybe meeting your husband is what really helped you get it all together finally A Yeah when I met my husband my world truly did change He was the first guy that I could be with where I could show all of my weaknesses and he still he loved me and he accepted me I remember the first night that we went out we talked until 3 in the morning and I told him about my cocaine addiction I didn t tell him about my sadness then and my depression But I figured hey if I could tell him about the coke and he was still listening this was a good thing So we dated for three years It was a very very rocky relationship It was very I was very high and very low all of the time I would explode over the smallest little things I would just go crazy There s a name for this Q So even though you really were quite happy and had kicked the drugs you still had these mood cycles and could get set off and then A There was one evening where it was so so bad I believe that this was after we were married and we were living in an apartment on Wilshire Boulevard in a high rise And I wanted to jump off the balcony I wanted to end my life I just had this pain that wouldn t go away I couldn t understand it And even though I had met the most wonderful man in the entire world I couldn t figure out why I was still so sad I couldn t figure out why I was crying myself to sleep every night and that my pain wasn t going away Q I think you had a girlfriend She felt you were depressed and there are things to do for it A Right And I was just like Oh my gosh there s a name for what I have and you can see that maybe I do have a problem but it s okay and that I can go somewhere and get some help and actually feel better I was amazed I mean people did not talk about depression at all And if you did hear about somebody that was depressed there was a stigma attached to it And you kind of would look down upon them like Why are they depressed You know they should be able to fight this Q Buck up yeah A They should be able to work it out and figure it out themselves Finding help hope Q

    just meditation It was creating stillness and looking at how I was brought up You really have to find all the pieces of who you are You look at your story and you tell your story in order to let your story go Silence really helps you get to a place where you can process who you are and let go of the things that are kind of constant reruns in your head those looping thoughts that one gets The voice that is you I ve talked about the small voice that doesn t give us a break in the back of our minds That voice is not the truth of us It s just a lie that we tell ourselves We can break that spell by becoming conscious of it You can do it through therapy I did it through yoga and learning to really sit with myself and be truthful But if you need a therapist if you need to talk to somebody to get there absolutely It s not easy I did a lot of journaling and self exploration Once you start opening up to that part of yourself and hear the voice that is not you and distinguish the voice that is not you from the voice that is you it s really liberating Some people may need help That s why I did the movie Running From Crazy that s why I write books to say Here are some tools to help you understand yourself and find a place of balance Trusting yourself Even before leaving my very long marriage and meeting Bobby I was ready to be myself I still felt like there was somebody outside myself who was going to have an answer for me What Bobby did for me sometimes you do need a person to go Hey you re fine the way you are It happened when we were rock climbing one day I was always saying Thank you for this thank you for that I appreciate it so much He said Stop thanking me You re making the choices You re doing this And it was such a metaphor for my life That was the shift in my self esteem I realized I was able to give myself what I need I knew that intellectually but I hadn t really embraced it fully I feel strong as me and I can honestly say I like me but it s been a very long journey And I think it s a long journey for many people Doing the work Finding balance is an everyday journey I work on my stuff every day It doesn t mean that because I have a scheme or because I m balanced or because I feel good I don t have moments when I m sad and need to remind myself that s not who I am We re human beings and we work all the time to create balance and harmony within ourselves People think
