    pacing fidgeting or wringing their hands Panic attacks or other types of anxiety may occur within or independently of episodes of depression If anxiety symptoms occur only during depressive episodes and subside as the episode resolves they likely represent another feature of depressive illness rather than a separate disorder The latest edition of the Diagnostic and Statistical Manual of Mental Disorders DSM 5 the standard reference for diagnosing a psychiatric illness acknowledges this important aspect of depressive episodes Someone with a diagnosed mood disorder either depression or bipolar disorder may receive the add on description with anxious distress known as a specifier if episodic symptoms include feeling tense unusually restless on the edge of control or afraid that something awful might happen If prominent anxiety symptoms occur at times when clusters of depressive symptoms are not present that probably indicates a separate anxiety disorder such as generalized anxiety disorder panic disorder agoraphobia post traumatic stress disorder or obsessive compulsive disorder Major depressive episodes may develop after a specific anxiety disorder is already established and anxiety symptoms often worsen when this happens How would my treatment be different When panic attacks or other severe anxiety symptoms accompany depressive episodes depressive symptoms in general tend to be more severe the episode often takes longer to r and future episodes may be more frequent The tendency toward longer and more frequent depressive episodes seem to increase as the scope and intensity of co existing anxiety symptoms increase The current pharmacotherapy for anxious depression does not differ in any proven way from that of non anxious depression though anxious depression often proves somewhat more resistant to treatment with medication Psychotherapeutic approaches such as cognitive behavioral therapy CBT or acceptance and commitment therapy ACT and other mindfulness based techniques appear to be effective for both
