  • Day 2 - Community Health Volunteer Class  - Patle Village Clinic - Chyangba village Clinic - Chyangba Village Medical Trek 2014 - Daily Dispatches
girls who were in the class yesterday skipped school there s a six day school week Sunday through Friday to come back today Their notes weave through English letters and Sanskrit board copied dates about John Snow and the London cholera epidemic next to summaries of immunizations in Nepali I didn t have a lecture today so I wandered around Khamding read napped and prepared for my class tomorrow on high altitude medicine I joined Sharon and Jennifer and Kunga who was translating at the grade school in Khamding ages three to 14 where Jennifer was teaching the youngest groups the English alphabet and playing games I went into a class of 13 year olds where the headmaster of the school or at least the man who acted liked the headmaster Nawang s uncle whom I d examined in clinic asked me through Kunga what I would like to teach What I would like to teach I hadn t prepared anything I thought I was just visiting I wasn t qualified to teach anything The class said they wanted a health class so Sharon and I bumbled through I did most of the bumbling a basic food groups talk until Yvonne showed up with Chongba and bilingual cards with food names and the class sorted them into proteins carbohydrates dairy and fruits and vegetables It was strange the way the teachers stepped back when we were there We didn t want to impose ourselves on these classes but ended up being pulled in by the few faculty members In the afternoon after learning about Ang Phula s our fearless chief and now head translator it s unclear how much he s directly translating and how much he s telling jokes experiences with high altitude illness he has a lot of

or intrigued by the notion of bloody diarrhea and seemed a bit queasy as they learned about liver failure Any discomfort however was defused as the class mixed their own rehydration formula which Karma spoon fed to a severely dehydrated child aka Stephanie Afterwards Jennifer covered the importance of immunizations and how to keep immunization records complete with illustrations The class nodded their understanding approval as Jennifer made a rather impressive white board sketch of a syringe Finally Max topped things off by giving an entire lecture on preventative medicine without referring to any notes His intensive Boards studying is definitely paying off Overall we had 24 people show up for the classes 21 women and 3 men I think we were all really pleased with the amount of people that came from neighboring villages eager to learn from us for this is the most important part of the entire program We were all super happy to see two 13 year old girls in the classes today and hopefully we can inspire them to continue to study health and eventually be a resource for their community As for the rest of us that didn t have to teach a lecture today we got a day off Most of us spent the day preparing for our upcoming lectures napping or exploring all that the one road of Khamding has to offer Adam Grace and I went searching for the tea shop that was advertised on a map we passed at the previous village We were hoping to find a store with an assortment of teas to buy but instead were taken into a random home and given a tea we hadn t had the pleasure to experience yet I believe it was nak butter tea and it was interesting to say

just as many patients if not more My role yesterday in clinic was to help each group with their technique and history taking Although it was a great experience to be able to work with each team and go through so many cases I was able to see my own patients this time which was very refreshing The practice style is different from what I am used to at our clinics back home Instead of having as much time as needed to see patients the seemingly endless line of patients means that there is more of a time factor to consider There is also a greater emphasis on the history and physical exam because of our limited resources Indeed the urine dipstick test a test which I used to skim over if I saw it in a patient chart back home has become essential in assessing the health status of a patients kidneys liver or general state of health No longer able to rely on a battery of blood tests EKGs and XRays I am learning to appreciate the art of practicing the fundamentals of medicine in which the history and physical become the main if not sole components of diagnosing a patient s ailments I would also like to take the time to praise my fellow undergraduate colleagues on how much they have learned in the past couple of days They are all extremely professional and kind in the way they interact with patients and have gained the skills that we learn in medical school in many months I am sure they are finding the opportunities to interact with patients to be very rewarding and I hope they are finding this experience invaluable in determining what they hope to accomplish in their career I would also like to also

phrases in order to be able to comfortably communicate with our patients Other than the introductory What is your name or My name is we even took the opportunity to ask our guides how to say key phrases like Where is your pain Please relax Deep breaths and quite a few more Who knows by the end of the trip we might just be pros at the language Upon arrival we were welcomed by an outstanding game of rainy day volleyball in the school courtyard Seeing as to how everyone else was able to enthusiastically embrace the weather we regained our energy and began to set up as well Plus the students of the school were so playful and adorable that we couldn t help but feel reinvigorated by their smiles and laughs Soon schoolhouse tables were moved benches were shifted and we had ourselves a makeshift clinic Spread out amongst 4 classrooms for triage exams and supplies we readily accepted our first patients People trickled in slowly at first but before we knew it there formed an expansive line of women children and elderly all looking forward to the exciting opportunity As we got into the rhythm of taking histories performing exams and chasing after Dr Murphy to do a final check we had already reached a total of twenty patients by lunch Speaking of which we had an aggressive lunch during which we quickly inhaled our food within 5 minutes in order to get back into the action Now THAT is dedication By the end of the day we garnered so much interest that some patients were soon given wait list numbers to return for tomorrow We saw a total of 56 patients and experienced a diverse list of health issues ranging from farmers joint pains to liver

To temper the frantic weather blogging that s come before me I ll let you know that the trek was happily uneventful We only tasted Nepal s monsoon downpour sporadically Leeches were a surprise not a habit We have all taken happy advantage of the opportunity to get to know each other and share our stories Altitude didn t strike us all too hard Jennifer s family she was hit with dizziness at our highest altitude and the Sherpas carried her down to Chyangba she feels almost completely well now and she ran around chasing the schoolchildren today We had camped downhill from a Buddhist temple the night before Though the morning mist stopped us from seeing Everest we paid our respect to the Buddhist culture and stopped at the temple before the hike down into Chyangba Ba Me I took pictures for you and I paid my respects as you have raised me to I think of you every day Now we are here The mist clears every once in a while to show us the terraces valleys and rivers below We can see their view from our tents We brushed past small corn fields and houses spread quarters

was a hard hike we were able to see some great views It was as if we were standing on top of the clouds After breakfast we hiked through cow pastures and made our way towards lunch After lunch we hiked towards our campsite for the day Nima of course charged ahead and kept track of everyone with his sonar hearing Grace Yvonne and Kim got lost and met a mysterious boy who will haunt Yvonne tonight in her dreams Apparently the boy insists that he didn t see them even though he clearly did When we got to the campsite we all sat down for tea and DJ Ngawang took out his phone to play some music We also learned that Phula s nickname Jennifer got a little high altitude sickness and the amazing guides carried her up to Peaky Peak and then back down the mountain and all the way to the campsite My respect for the guides has increased infinitely and they all get an A and a standing ovation for the day I feel so privileged to be hiking with a group of people with such big hearts and I ve learned so much from them
